You can also report your passport as lost or stolen by using Form DS-64: Statement Regarding a Lost or Stolen Passport.  Mail this form to:

If your passport is lost or stolen overseas, contact the nearest U.S. Embassy or Consulate.  

If you are in the United States, you must submit two forms to replace your lost or stolen passport.  Submit Form DS-11: Application for a U.S. Passport and Form DS-64: Statement Regarding a Lost or Stolen Passport in person to a Passport Agency or Acceptance Facility.  

If you find your passport after having reported it lost or stolen, submit it to the address listed above.  The office will cancel your request or destroy your passport.  Once a passport is reported lost or stolen, it cannot be re-validated.
