The Hague Adoption Convention requires that a report is filed for every outgoing adoption from the U.S. and any non-Convention cases. You must report with the following information to the Department of State’s Office of Children’s Issues within 30 days:
- Child's name, date of birth, and place of birth
- U.S. State from which the child is emigrating
- Country to which the child is immigrating
- U.S. state where the final adoption is taking place
- Government office’s name, address, phone number, and other contact info
