Why should I create a Living Will?

A Living Will lets you.. 

• Name a specific person to make health care decisions for you when you can't make them for yourself.
• Plan for surgery or hospitalization.
• Assist your loved ones with difficult decisions.
• State your wishes so that it is more likely that they will be carried out.

What do I need to get started?

To get ready to write your Living Will, you may want to gather the following:

• Name and address of the person or persons who will make health care decisions for you if you are unable to do so.
• Your name, address, social security number, and date of birth.
