- The name and address of the corporation (to transfer stock in a closely-held corporation).
- The name of the revocable living trust to receive the account or security.
- The names, addresses and Social Security Numbers of the owners of the accounts or securities.
* Names and addresses of any non-owner, currently-acting trustees. Federal Identification Number of the trust, if the owner is not the trustee.
