To obtain the appropriate address, refer to a telephone book or call directory assistance for the locale where the event occurred. For your reference, libraries have copies of phone books from other cities. When you call the local records office, you should request the following information: - the appropriate address;
- the applicable fee and acceptable form of payment; and
- information regarding to whom payment should be made.
