When describing the incident in this report it is important that you are as specific and detailed as possible. You should include all the facts available to you. To ensure you consider all aspects of the incident, you should include:
Pre-Incident Details: Include a description of the activities being completed leading up to the incident. For example, if you were involved in a fall from a ladder where you subsequently broke your leg, you could state, " On the morning of my fall I was repairing a roof at 123 Oak St. I was responsible for carrying shingles up to the roof before we started the repairs. I was using a 10 foot ladder propped against the side of the house to climb up to the roof while carrying the shingles" .
Incident Details: Include a detailed description of the incident. For example, �On my 3rd trip up the ladder I was carrying 2 packages of shingles over my right shoulder. I was three quarters of the way up the ladder when I lost my footing. When I stumbled the ladder began to shake and then began to fall to the right. I dropped the shingles and attempted to let go of the ladder. I landed on my right side and the ladder fell across the top of my right calf" .
Post-Incident Details: Include a detailed description of any injuries suffered during the incident. For example, "My right side was bruised and sore from the fall. I also have a broken left leg that was caused when the ladder landed on top of my leg" .
It is also important for you to detail any objects or substances involved in the incident. For example, if an employee sustains a burn while working with a chemical, the name of the chemical should be included in this report.
