Free Request to Remove Name from Direct Marketing List

This document is used to request removal of your name (and address or phone number) from the lists of a direct marketer and/or the Direct Marketing Association (DMA).

Use the Request to Remove Name from Direct Marketing List document if:

You want to contact a specific direct marketer to be removed from a mailing or calling list.
You want to contact the Direct Marketing Association to be placed on a "Do Not Call" and/or "Do Not Mail" list.

Request to Remove Name from Direct Marketing List Signing Instructions The Direct Marketing Association Request to Remove Name from Direct Marketing List The Request to Remove Name from Direct Marketing List is used to request removal of your name and address or name and phone number from a direct marketer's list. It can also be used to contact the Direct Marketing Association to have your name placed on a "Do Not Call" and/or "Do Not Mail" list. Federal regulations provide that a telemarketer cannot call a consumer's home before 8:00 a.m. or after 9:00 p.m. Further, telemarketers must honor a consumer's request not to be called again. Telemarketing companies are required to have written policies regarding the maintenance of "Do Not Call" lists. If a consumer asks the telemarketer not to call the consumer, then the telemarketer cannot call the consumer for up to 10 years. Signing Instructions The Request to Remove Name from Direct Marketing List should be signed, but it is not necessary that the signature be witnessed or notarized. Retain a copy for your records. The Direct Marketing Association The Direct Marketing Association ("DMA") is a telemarketing industry group that collects names to be placed on "Do Not Call" and "Do Not Mail" lists for each individual company that belongs to the Association.
