Free Request to Cancel Credit Card

If you are making the request on behalf of someone else, you may be required to show proof of your authority to make the request. For example, if you are making the request on behalf of a person who has died, then proof of your authority may be required. Required proof might include a court document that appoints an �executor�, �administrator�, or �personal representative�, to act on behalf of an estate. If such a document is not available, then some other type of documentation may be required in order to complete the process. Similarly, if you are managing someone's finances under a power of attorney document, you may need to provide a copy of the power of attorney document.

The Request to Cancel Credit Card is used to request the cancellation of a credit card. It may be a cancellation request for your credit card or you may be making the request on behalf of someone else, for example, on behalf of a card holder who has died.

1. The Request to Cancel Credit Card should be signed, but it is not necessary that the signature be witnessed or notarized.

2. If you so indicated, be sure to include evidence that you are the executor, personal representative, surviving spouse, agent under a power of attorney, or other authorized person to make the request.

3. Retain a copy for your records.
