If applicable, the company may require that you include certain documentation verifying your identity. Contact the company to determine what additional information, if any, may be required.

Photocopies, not the original copies, of any relevant documents should be included with the letter (unless otherwise required by the company).

Federal and state legislation is moving increasingly in the direction of prohibiting the use of key personal identification information (e.g., your social security number) as your account number. Without waiting on such legislation, you should be proactive in limiting the use of your social security number to those situations where it is essential. For example, if a company uses your social security number as your account number, your are at a higher risk of becoming a victim of identity theft.

1. The letter should be signed, but it is not necessary that the signature be witnessed or notarized.

2. Be sure to attach any documentation, if applicable.

3. Retain a copy of the letter for your records. Keep the original copies of any enclosures, unless you are required to send the originals. If you send an original, keep a copy.

4. After a reasonable amount of time has passed, follow up to make sure that any request action was actually taken.
