Free Request for Monthly Statements

If applicable, the bank or credit card company may require that you include certain documentation. Contact your bank or credit card company to determine what additional information, if any, may be required.

Photocopies, not the original copies, of any relevant documents should be included with the letter (unless otherwise required by the bank or credit card company). If you suspect you may be the victim of identity theft, you should keep a copy of all written communications with the bank or credit card company and maintain a written summary of all oral communications.

The purpose of this letter is to request and authorize a bank or credit card company to re-issue monthly statements to you for a specific time period. Generally, you should be receiving your monthly statements regularly. However, if a statement was misplaced or misdirected, you should immediately request such statement so that you can review it and/or submit payment against it. Further, with the increasing risk of identity theft, you may simply want to review past statements for possible fraudulent activity, ensuring that all transactions match up with your own records or recollection.

1. The letter should be signed, but it is not necessary that the signature be witnessed or notarized.

2. Be sure to attach any documentation, if applicable.

3. Retain a copy of the letter for your records. Keep the original copies of any enclosures, unless you are required to send the originals. If you send an original, keep a copy.

4. After a resonable amount of time has passed, follow up to make sure that any requested action was actually taken.
