Free Report Lost or Stolen Passport

The U.S. Department of State will probably want to follow up with you if they need additional information. Therefore it is essential that all of the contact information included in your letter is accurate and up to date.

The U.S. Department of State may require that you include proof of your lost or stolen passport such as photocopies of a filed Police Report, etc.

Photocopies, not the original copies, of any relevant documents should be included with the letter (unless otherwise required by the U.S. Department of State). You should keep a copy of all written communications with the U.S. Department of State and maintain a written summary of all oral communications.

If your passport has been lost or stolen you should report it immediately to the U.S. Department of State to prevent fraud. The number to call is (202) 955-0430. In addition, you should file U.S. Department of State Form DS-64 Statement Regarding Lost or Stolen Passport at your local passport office or you may mail it to:

The U.S. Department of State may request additional documentation, but include copies of the following, if available:

• A copy of the Police Report.
  
• A copy of your driver's license or another form of identification (e.g., a social security card).
  
• A copy of any other documentation or information about the lost or stolen passport and how you became aware of it.

After you have reported the passport lost or stolen, it will be invalid for any further use by anyone. If you subsequently find or recover the passport you must immediately return it to the U.S. Department of State, Washington, D.C. 20522-1705 or to the nearest passport agency.

1. The letter should be signed, but it is not necessary that the signature be witnessed or notarized.

2. Be sure to include the original copy of U.S. Department of State Form DS-64 Statement Regarding Lost or Stolen Passport. Send copies of any other documents instead of originals, unless you are required to send an original.

3. Retain a copy of the letter for your records. Keep the original copies of any enclosures, unless you are required to send the originals. If you send an original, keep a copy.

4. Although not required, sending the letter in a manner that can be tracked (e.g., overnight courier or certified mail/return receipt requested) is highly advisable. It may assist you in the event you need evidence of receipt of the letter.

5. After a reasonable amount of time has passed, follow up to make sure that any requested action was actually taken.
