Free Confirmation of Enrollment

This letter can be used to request information from a college or university to verify that a student is enrolled as a full-time student in order to prove eligibility for health insurance benefits.

Use the Confirmation of Enrollment document if:

Your health insurance company has requested proof of the student's enrollment status as a full-time student before paying for medical services.
You need to provide proof of enrollment for a scholarship grantor or other requestor.
