Birth Certificate Request Letter Contact Information Fees Names of the Parents Signing Instructions Vital Statistics Offices Birth Certificate Request Letter PLEASE NOTE: To complete your birth certificate request, you will need to print and mail or fax the Birth Certificate Request Letter you create. An official certificate of every birth is on file in the vital statistics office in the state where the birth occurred. Use this letter to request a copy of a birth certificate from a state's vital statistics office. Contact Information PLEASE NOTE: To complete your birth certificate request, you will need to print and mail or fax the Birth Certificate Request Letter you create to the appropriate state vital statistics office. Once your letter is received, the vital statistics office may have questions about some of the information that you have provided or may require additional information. Therefore, your letter should explain how you prefer to be contacted. Fees Because the fees are subject to change, you should call the vital statistics office for your state to verify the current fee, the acceptable form of payment, and the name to whom payment should be made. A cash payment is not recommended because it does not provide proof of payment. You should enclose a self-addressed, stamped envelope with your letter. Names of the Parents Vital records are usually stored and accessed according to the names of the parents. You need to provide the names of the parents, including the mother's maiden name (if known). If the person was adopted, enter the names of the adoptive parents, and the adoptive mother's maiden name. Adoption records, including the names of biological parents, are usually sealed, and the child is issued a new birth certificate containing the names of the adoptive parents. This becomes the child's official birth certificate. If you do not know one of the required names, enter "unknown," but check with the state's vital statistics office to see if additional information may be required. Signing Instructions The Letter should be signed by you. Retain a copy of the letter for your records. The address may have changed since the publication of this list, so it's a good idea to call the office first to confirm the address. For New York birth certificate requests, please note: the birth records for New York City are kept at the Bureau of Vital Records in New York City; the records for the rest of the state are kept at the State Department of Health in Albany.
