Free Affidavit of Same Name

The Affidavit of Name Change can be used to verify the identity of a person who has assumed a new name or uses multiple names. The person making this affidavit (preferably a blood relative) must have personal knowledge of the applicant's use of both names. Affidavit of One and the Same and Affidavit of Same Name are other names for Affidavit of Name Change.

Use the Affidavit of Same Name document if:
  • You have changed your name in the past and now a party you are dealing with wants assurances that the name change is proper and there is no chance of impropriety.
