Free Affidavit of Name Change
You may not be James Bond, but you may use names or aliases other than your legal name. An Affidavit of Name Change can help you prove you're who you say when you've changed your name, but haven't made it official. If you have been using more than one name, create Affidavit of Name Change to verify your identity.

It will work for anyone who has used a name that has not been changed officially by a court or changed through marriage. For example, you can use this document if you have recently been married, divorced or formally changed your name, but need additional verification of your name change. Another person, preferably a blood-relative, who has personal knowledge of you using both your legal name and your chosen name, will fill out this Affidavit.
